Accreditation for Service and Repair Shops

Service and repair shops dealing with motor vehicle, heavy equipment; engine and engineering; electronics, electrical; air conditioning and refrigeration; office machine; data processing equipment; and medical/dental are required to apply for accreditation with the DTI.

Description of Service             :               Issuance of Certificate of Accreditation for Repair Shops

Clients                                     :               Service and Repair Shop Operators

Requirements                          :               For both Original and Renewal Applications

1.        Duly accomplished Application Form

2.        Copy of Comprehensive General Liability Insurance Policy and Receipt of Payment.  Insurance period must be at least up to the end of the year (December 31)

3.        Warranty

4.        List of Tools and Equipment & corresponding value

5.        List/names of shop employees (name/s of person) & Position

6.        Size of shop/stall

7.        Photo of shop – 2 copies (front & inside)

8.        Shop layout

9.        Organizational chart

10.     Copy of Business Name Certificate of Registration

11.     Certified True Copy (by Corporate Secretary) of SEC Certificate and Articles of Incorporation (if Corporation)

12.     TESDA Certificate of Competency for Technical employee/s (for 3 to 5 STAR Category only)

13.     Performance Bond ( for 3 to 5 STAR Category) P50,000.00

14.     Dealership Agreement for 5 STAR Category only

15.     TESDA Certificate of Competency for Technical Employee/s (For accreditation of  REFRIGERATION and AIR CONDITIONING (RAC) and MOBILE/AUTO AIRCONDITIONING(MAC) regardless of star classification
